phase III

EMPOWER

The beginning of possibility and opportunity.

Engaged by a vision of hope for their future and inspired by their goals, clients leave inpatient treatment and move to one of our transitional living facilities. This is where they become more independent and continue the pursuit of their aspirations. Addiction treatment and life skills development will continue in the NuLife outpatient program, with a focus on practical application and living in sobriety. Phase III empowers clients to experience recovery and the joy of a life beyond addiction while still under NuLife’s structured care.

Clients will continue to build and strengthen their support systems through alumni activities, 12 step fellowship, and personal recovery coaching. They learn how to safely pursue their educational, employment, and vocational goals without compromising their commitment to long term recovery.

the focus

Esteem needs: Personal value, competence, personal achievement, appreciation and recognition from others, and self-actualization. Safety needs: Transition to a stable, secure and predictable environment, self-sufficiency, structure, accountability, personal responsibility, and support system utilization.

the goal

Facilitate successful integration into their respective families, homes, and communities by providing a clear path to long term recovery – a NuLife full of reasons to stay in recovery.

Phase III: CARE COORDINATION SERVICES

In the last phase of care, we promote personal responsibility and accountability,
increased independence, and a safe, strategic transition to normal life.

Phase III Clinical Care shifts the focus to application and the experience of living in sobriety. During this Phase, clients are supported by Clinical Therapists as they transition to the outpatient program, where they put their newly acquired coping skills to use in real world situations.

The Case Manager continues to oversee the clients Personal Care Plan, sobriety support, life management skills, and family relationship building, while adding a new focus on educational goals, employment, and transitioning to independent sober living.

Phase III Families fully understand the disease of addiction and its treatment. They’ve learned how they play a role in the recovery process, and begin to see changes in their loved one. This gives them strength and hope – which is invaluable to other families in the family program.

Phase III clients continue with their Twelve-Step Guides and attend structured step meetings where they are held accountable to finishing the work. As clients progress through the steps, they clean up the past, take responsibility for their lives, and become step guides themselves.
